Submitting a complaint about a DANB Certificant. DANB Certification represents excellence in the oral healthcare profession and is a source of pride for those who achieve it. It is important for the Dental Assisting National Board, Inc. to learn about any individuals who are falsely claiming to be DANB Certified or practicing in the profession in a manner that would not be in line with the level of excellence that the credential represents.

Any person concerned about the practice of a DANB Certificant, can direct their concern to Cynthia Durley, Executive Director, Dental Assisting National Board, Inc., 444 N. Michigan Ave., Suite 900, Chicago, Illinois 60611. A Complaint Information Release Authorization Form must be submitted with any correspondence.



This listing below of DANB’s Disciplinary Actions contains sanctions meted out by DANB to Certificants, Certificate of Competency holders (RHS, ICE), or others for violations of DANB policy, such as misuse of DANB credentials and irregular behavior associated with DANB exams.

Each case contains the following information: Individual identification by state, grounds for disciplinary action against the individual (based on DANB’s Discipline Policy), the decision of DANB’s Informal Review Committee (made up of members of DANB’s Board of Directors), and the sanctions imposed by the Committee.

As the nationally recognized testing and credentialing agency for dental assistants, DANB has a responsibility to its stakeholders to ensure that DANB credentials are only used by those who have earned them. If you know of anyone in violation of DANB policies, you should submit a Complaint Information Release Authorization Form to DANB. (See above.)

Case 1: Complaint: Fraudulent DANB CDA certificate from a lapsed DANB Certificant
State: Tennessee

Circumstances: Individual was accused of manufacturing a fraudulent DANB CDA certificate and of using that fraudulent certificate to misrepresent herself to her employer as a CDA. The CDA certificate that she provided to her employer bore another individual’s certification number.
Grounds for Action: Obtaining or attempting to achieve a passing score on an exam or to obtain certification or recertification by fraud, deception, or when ineligible, or assisting another person to do any of the above; reproducing, distributing, or using a fraudulent or otherwise unauthorized DANB certificate, or assisting another person to do any of the above; and failure to cooperate reasonably with a DANB disciplinary investigation.
Decision: Guilty
Sanctions: Prohibit reinstatement of respondent’s CDA credential for a period of five years; prohibit the respondent from taking any exam administered by DANB for a period of five years; require respondent to provide proof of completion of an ethics course before applying to reinstate (reinstatement decision, including fees to be owed, will be made at the time of application for reinstatement, after expiration of sanction period); notify legitimately interested parties, including respondent’s employer and state dental board in respondent’s state, of DANB disciplinary action against respondent.

Case 2: Complaint: Fraudulent DANB CDA certificate from a lapsed DANB Certificant
State: Ohio

Circumstances: The individual was accused of attempting to obtain recertification by fraud or deception. She submitted an altered wallet card to DANB in an attempt to prove that her Certification had expired 31–32 months earlier, when, in fact, her Certification had been expired for over six years.
Grounds for Action: Intentional misstatement of material fact or deliberate failure to provide relevant information to DANB during the application process or at any other time; obtaining or attempting to achieve a passing score on an exam or to obtain certification or recertification by fraud, deception, or when ineligible, or assisting another person to do so; and reproducing, distributing, or using a fraudulent or otherwise unauthorized DANB certificate, or assisting another person to do any of the above.
Decision: Guilty
Sanctions: Prohibit respondent from reinstating her lapsed DANB credential or to apply for or take the CDA examination or any other DANB-developed and/or administered examination for a period of at least five (5) years from the date of notification; require respondent to provide documentation that she has completed a course in professional ethics prior to accepting an application for any DANB examination in the future; instruct respondent that she is to immediately cease and desist from using any unearned, unauthorized DANB credential; warn respondent that, should she commit any other acts of fraud or deception involving a DANB wallet card or any other DANB-issued document in the future, DANB will consider immediate legal action; and notify all legitimately interested parties, including regulatory boards, dental associations, dental assisting associations, employers, etc., of DANB’s findings and disciplinary action.

Case 3: Complaint: Fraudulent DANB CDA certificate from a failing CDA exam candidate
State: North Carolina

Circumstances: The individual was accused of submitting a forged and falsified letter to DANB in furtherance of her contention that she was in possession of and had shown correct identification at the time that she was denied admission to her appointment.
Grounds for Action: Intentional misstatement of material fact or deliberate failure to provide relevant information to DANB during the application process or at any other time.
Decision: Guilty
Sanctions: Prohibit the respondent from taking any DANB Exam for a period of five years; require respondent to provide proof of having completed a course in professional ethics before accepting any exam application from respondent; notify the bureau of radiological health in the respondent’s state that the respondent has been disciplined by DANB for submitting a falsified document; notify the board of dentistry in the respondent’s state that the respondent has been disciplined by DANB for submitting a falsified document; notify the ID card office at the respondent’s university that the respondent has been disciplined for submitting a falsified document.

Case 4: Complaint: Assisting in creating a fraudulent DANB CDA certificate
State: Tennessee

Circumstances: A current DANB Certificant was accused of assisting another individual to manufacture a fraudulent DANB certificate. This respondent’s certification number appeared on a fraudulent DANB certificate that another individual submitted to her own employer as proof of DANB Certification.
Grounds for Action: Reproducing, distributing, or using a fraudulent or otherwise unauthorized DANB certificate, or assisting another person to do any of the above.
Decision: Guilty
Sanctions: Revoke respondent’s CDA credential; Prohibit the respondent from taking any exam administered by DANB for a period of five years; require respondent to provide proof of completion of a course in professional ethics before applying for any exam in the future (determination as to eligibility for future exams will be made at the time of application, after expiration of sanction period); notify respondent that DANB will consider immediate legal action if respondent should attempt to reproduce, distribute, or use a fraudulent or otherwise unauthorized DANB certificate in the future or assist any other individual to do so; Notify legitimately interested parties, including the state dental board in respondent’s state, of DANB disciplinary action against respondent; must immediately return to DANB any and all originals and copies of DANB certificates, wallet cards, and any other documentation or materials that indicate that he or she is Certified by DANB.

Case 5: Complaint: Fraudulent documentation related to candidate identification
State: New Jersey

Circumstances: The individual was accused of manufacturing a fraudulent DANB wallet card and using such fraudulent card to misrepresent herself as a CDA to her employer.
Grounds for Action: Reproducing, distributing, or using a fraudulent or otherwise unauthorized DANB certificate, or assisting another person to do any of the above.
Decision: Guilty
Sanctions: Notify the state dental board that the respondent is not a CDA, but has misrepresented herself as a CDA; notify the state dental board that the respondent has been disciplined by DANB; notify the employer requesting the original credential verification that the respondent has been disciplined by DANB; prohibit the respondent from taking any exam administered by DANB for a period of five years; and prohibit respondent from taking any DANB examination in the future unless the following conditions have been met: Respondent must immediately send DANB any and all copies of the fraudulent wallet card; and Respondent must recant in writing any statement or suggestion that she is a CDA that was previously made to any employers, potential employers, or any other entity, and must provide DANB with copies of such written recantation; and Respondent must immediately provide DANB with documentation that she has notified her current employer that she is not a CDA; and she must notify all employers during the ensuing five-year period that she is not a CDA and that she has been disciplined by DANB, and she must provide proof to DANB of such notification; and Respondent must provide proof of having completed a course in professional ethics.
